The UL Hospitals Group has initiated an internal investigation following the recent passing of a 16-year-old girl at University Hospital Limerick (UHL) three weeks ago. Reportedly, on the day of her passing, January 29th, the hospital accommodated 113 patients on trolleys in corridors and wards. The deceased girl is understood to be from Co. Limerick.
This incident marks the second internal inquiry into the death of a teenage girl at UHL within the past two years. In December 2022, Aoife Johnston (16) from Shannon, Co. Clare, tragically succumbed to meningitis after enduring a 12-hour wait on a trolley in UHL’s Emergency Department. Following an inquiry into Aoife’s death, the HSE issued an apology to the Johnston family, citing “serious failings in the care” provided to Aoife.
In light of this recent tragedy, an inquiry has been initiated to investigate the circumstances surrounding the second teenager’s passing and the care administered to her at the hospital on the day of her demise. The UL Hospitals Group has extended its heartfelt condolences to the bereaved family, acknowledging their profound loss.
A preliminary assessment, conducted in adherence to the HSE Incident Management Framework, has been completed, with the findings to be shared with the family pending the results of a post-mortem examination. Under this framework, incidents resulting in patient harm, such as this, are categorized as Category 1 incidents and warrant thorough review and investigation.
